This page contains links to the various forms currently in use by the National Association of USS LCS(L) 1-130.  These forms are available for local printing on your printer.  You can print any of the forms, then you can fill them out for mailing.

The LCS(L) 102 Retrieval Donation form can be accessed by clicking on 102 Retrieval/Maintenance Donation Form and then printing the page.  Please send the form and renewal fee to Dennis Steenbergen, 7345 W. Lakeside Drive, Littleton, CO 80125.  It is true that the LCS(L) 102 is docked at Mare Island, but the National Association of USS LCS(L) 1-130 still needs to pay some final charges to the National Air Cargo company.  Your donations will be greatly appreciated.
